Installing and dismantling tower cranes in urban construction sites are crucial but extremely hazardous tasks, particularly in high-rise building projects in busy downtown areas. These operations are prone to accidents, often resulting in fatalities, delays, and structural damage. This paper investigates the causes of these accidents through analysis and focus group interviews (FGIs) with crane personnel. Findings show that 68.4% of fatal construction site accidents occur during crane operations, primarily due to non-compliance with procedures and unsafe worker actions. Other risk factors include worker competence, stakeholder responsibilities, crane component integrity, and work conditions. These insights can help regulators and practitioners improve safety standards, refine protocols, enhance training, foster stakeholder collaboration, conduct regular inspections, and optimize work conditions, ultimately reducing crane-related accidents and ensuring safer construction environments.
